CEU REPORTING The Continuing Education/Formation Report Form is due at the same time as the annual clergy evaluation (due by May 25 each year) and then reviewed as part of the clergy one-on-one. It is to be filled out and signed by the Pastor, the chair of the S/PPRC and the District Superintendent. Word, to fill in on your computer: Continuing Education/Formation Report Form in MicWord (Please make four copies of the CE/F Report form: One copy kept by the Pastor, One copy for the S/PPRC Chairperson, and Two copies sent to the District Superintendent (District Offices please forward one copy to the CCEF Committee: Rev.
